Lock Out Tag Out Procedures

Only authorized employees and vendors are allowed to repair or maintain the machines. The following steps below are to be followed:

Before performing any repairs or maintenance work on the machine/equipment:

  1. COMPLETE THE LOCK OUT TAG OUT FORM BELOW before starting.
  2. PREPARE: Identify all sources of energy - electrical, pneumatic/air pressure, steam, hydraulic, mechanical, etc. and locate isolation devices. See the photos above.
  3. NOTIFY: Inform all affected employees of the work and the machine involved.
  4. SHUT DOWN MACHINE: Turn off the machine properly to prevent hazards.
  5. ISOLATE ENERGY SOURCES: Use breakers, valves, switches to isolate the machine from its energy source.
  6. LOCK & TAG: Authorized employees must apply personal locks and tags to energy isolating devices to prevent re-energization. See the photos above.
  7. RELEASE STORED ENERGY: Deenergize/dissipate/remove/restrain any remaining/residual energy stored in the machine.
  8. VERIFY: Ensure no person is around to test the machine is unpowered and cannot start by trying the start button. Confirm that the machine is fully off and safe to work on.

After the work is done:

  1. Remove any tools or parts and put safety covers or guards back in place.
  2. Take off the lock(s) and tag (s).
  3. Re-energize the machine.
  4. Notify all affected employees that the machine is ready and safe to use again.
